Replacing/recovering a lost password/username is a three step process!!!

1. Type your username OR your e-mail address and click the send button. An email will be sent to you with your username and a 'Confirmation Code'.
2. After you receive the confirmation code, type your USERNAME and the 'Confirmation Code' into the form and click the send button.
3. When this form is submitted with the confirmation code, a new password will be generated and emailed back to you. Copy the new password and use it to log in.
